With voluntary alignment, the Medicare patient is able to self-elect their PCP. If that PCP is also on an MSSP participant list, then that patient will be aligned with that ACO.
With claims-based alignment, the beneficiary is aligned if they received a plurality of their primary care services (PCS) from an MSSP ACO Provider.
