Overview
On the Strategize page, the Patient Paths tab provides a visual representation of patient care pathways through a network of facilities. Each "layer" as you move out from the center is a subsequent step in the care path for patients from the selected facility; each ring is a step in a progression of care. You can also view each segment in each ring as a different location in a stream where you can look upstream to the source of patients for your "current" destination, or downstream to subsequent locations of patient care.
All metrics - counts and percentages - for any segment will represent a portion of the full patient population in the "upstream" location.
The search field allows you to find any hospital by name or NPI. When you find the hospital you want, click on it and the Strategize page will populate with data for that hospital.
The Filters selector allows you to narrow the patients included in the metrics on the page in a boatload of ways. (See below.)
The Patient Paths Chart is a visual representation of the patient paths related to your chosen facility. Each circle represents a subsequent step in the progression of care. You can hover over any segment of the chart to identify what each segment represents. Clicking on a segment will update the tables on the page to represent the metrics for that chosen segment.
Path Detail table - This table show the metrics for the selected segment and ring you have chosen along with any "upstream" care settings. In this example, we have selected the segment for home health, which is in the second ring. The table shows the Source Inpatient counts and percentages, and the relative home health metrics for the patients from the Inpatient facility. As you click on additional rings in the care path, more rows will be included in this table. You can think of this table as the view of the patient path for your current location looking upstream.
Patient Destinations - This is the breakdown of the patient destinations for the chosen facility segment. The metrics are calculated from claims only for the selected facility type in the segment you clicked. The destinations are sorted by patient count at each destination. You can think of this as the view downstream from you selected segment.
You can choose how many destinations are shown in this table by using the dropdown beneath the table:
Filters
If you want to view a limited patient population, click on the filter drop down and select from the numerous options. See below for more details.
- All Patients - As you might guess, this will include all patients treated by the selected facility during the reporting period.
- Total Days from INP Discharge - This will limit the sunburst chart to claim events that occur within the selected period of time after INP discharge.
- Care Lapse Constraint - Use this filter to view claims that occurred within 30 days of each other. If there is a 30-day lapse in care, the rings will end.
- Acuity - Trella Health calculates three levels of patient acuity. For more information click here.
- MDC - You can select any of the Trella Health Diagnostic groupings - for more information on these groupings, click here.
Although the header in the selection says, "MDC," you should not think of the CMS Major Diagnostic Categories. Although there are some similar names, these groupings do not align with CMS MDCs. Click on the link above for more information.
