30 Day Readmission Rate

A readmission is a care event where a patient is admitted to any inpatient stay within 30 days of being discharged from an inpatient stay. The Trella Health Readmission Rate is a percentage measurement of readmissions against a select population. This rate can be calculated for either hospitals or post-acute agencies.

Top Destination

This is the home health agency that is treating the highest percentage of home health patients discharged from the selected facility.

 You can add this metric to your explore table using the "Column Metrics" tool.

Total Patient Volume

This is the count of distinct Medicare patients who received services at this facility during the most recent four quarters, separated by claim type. 

Patient Risk Score

Average Hierarchical Condition Category (HCC) Patient Risk Score for the selected hospital. The High/Medium/Low beneath the number is the Risk Category,  which is the categorization of this hospital's Patient Risk Score in comparison to all similar hospitals.
